A lemon is smutfic that, following the Citrus Scale, is the most citric, and thus furthest down on the scale. Lemons tend to have explicit sexual content, if not actual sex.
While some lemons do include the occasional original character or Mary Sue, they generally feature real people or canon characters.
The department that handles the most lemons and smutfic in general is the Department of Bad Slash. However, since missions are assigned by fandom, not rating, anyone can find themselves facing a fic with dirty bits in it.
While a fic being a lemon is not a charge, being a bad lemon is.
